Job Description

sician, our Speech Therapist offers essential consultancy in speech therapy. Your responsibilities include but are not limited to, the development and implementation of rehabilitation services for patients encountering speech, language, or swallowing disorders, coupled with maintaining detailed clinical and progress documents.

Qualifications:
• Advanced degree (Masters or higher)
• Current and valid BLS, ACLS, or PALS certification/license from the American Heart Association
• Current and active Speech-Language Pathologist (SLP) certification/license applicable in Illinois
• A minimum of one year experience with Post-Certification Supervision within an institutional setting

Key Duties:
• Devise, implement, and assess rehabilitation provisions for speech, language, and/or swallowing disorders.
• Perform in-depth evaluations of patients' speech and language capabilities through diagnostic tests; establish goals and predict recovery probabilities.
• Document patients' progress and report alterations in patients' conditions to the physician.
• Offer guidance to family members and healthcare team members on how to assist the patient in enhancing communication skills.
• Regularly re-evaluate the patient's progress and adjust the rehabilitation program accordingly.
• Create routine reports, as per Agency requirements, to be submitted to the physician and third-party payers concerning provided care.
• Facilitate necessary referrals to external audiology speech clinics and community-based services.
• Uphold comprehensive clinical and progress notations and comply with Agency policy for timely submission of all required paperwork.
• Establish and sustain contact with other community agencies for follow-up.
• Actively participate in in-services, case conferences, and staff meetings.
• Consistently enhance professional competency through in-service attendance and similar professional resources.
• Carry out any additional duties as assigned by your supervisor.

The work schedule corresponds to the assigned caseload. The role requires daily commute for meetings and visits to patients' residences, working in varied living environments.
