Job Description:
Part-Time Nights:
1600-0030 Wednesday, Thursday, and every other Friday, Saturday, Sunday
Provides referrals for patients within the agency and community. Works directly with patients, family members, physicians, and community providers to ensure collaborative and continuous care. Identifies and intervenes for suspected abuse, including sensitivity to age-specific concerns. Participates in multidisciplinary treatment team meetings, providing clinical input. Formulates discharge plans in collaboration with the multidisciplinary treatment team, the patient, and the family, ensuring outpatient appointments are scheduled upon inpatient discharge. Demonstrates working knowledge of the involuntary commitment process. Conducts comprehensive assessments to include risk assessment and establishes therapy treatment objectives. Administers individual, family, and group therapy sessions. Evaluates patient data; develops and implements treatment plans. Documents therapy observations and maintains records, adhering to confidentiality protocols. Evaluates patient progress and therapy intervention effectiveness. Other duties as assigned.
Knowledge, Skills, and Abilities:
• Ability to gather individual patient assessment data, including individualized treatment plans, and referrals [Required]
• Ability to analyze and assess data, techniques, methodology, equipment operations, and quality control to ensure that information is obtained and presented accurately [Required]
• Ability to master educational, counseling, and administrative principles, theories, techniques, and practices [Required]
• Ability to work collaboratively as a dynamic treatment team member [Required]
• Basic computer skills and knowledge of Microsoft Office [Required]
• Must be able to read, write and speak conversational English [Required]
• Suggest and participate in process improvement opportunities in the performance of duties [Required]
Education:
• Master's [Required]
Field of Study:
• in social work, psychology, or related field
Work Experience:
• 1+ experience in a healthcare setting [Preferred]
• 2+ relevant experience [Required]
Additional Information:
• Part-time Nights
• 24 hours a week
• Every Monday and Tuesday
• 7pm to 7:30am
Licenses and Certifications:
•Basic Life Support - CPR Cert (BLS) [Preferred]
