Position

Summary

Under the general supervision of the CMC Medical Director, the Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) provides comprehensive behavioral health services in an ambulatory clinic setting. This includes psychosocial assessment, individual and family therapy, treatment planning, coordination of care, and linkage to community resources. The counselor supports both behavioral and medical integration by collaborating closely with primary care providers and interdisciplinary teams to promote whole‑person care.

The clinician assists patients in managing mental health conditions, life transitions, trauma, and chronic disease‑related behavioral changes while maintaining compliance with state licensure, payer requirements, and MGRMC policies.
Required Qualifications
• Master’s degree in counseling, Social Work, or a related behavioral health field from an accredited institution.
• Current Arizona license as LPC, LCSW, or LMFT (independent level).
• Minimum of one (1) year of outpatient or integrated care experience.
• Current CPR Certification (AHA – Healthcare Provider).
• Arizona Department of Public Safety fingerprint clearance card (or eligibility to obtain).
• Proficiency in electronic health record (EHR) systems and basic computer literacy.
• Excellent written and verbal communication skills and ability to maintain composure under stress.
• Demonstrated ability to collaborate effectively with medical providers, families, and community partners.
Preferred Qualifications
• Experience in an integrated primary care–behavioral health model.
• Familiarity with Meditech or similar EHR system.
• Knowledge of CPT/DSM-5/ICD-10 diagnostic and billing processes.
• Two or more years of clinical practice in rural health or community behavioral health.
