Role Description
The Adult Mental Health Specialist is a full-time, on-site role located in Cleveland, TX. This role is responsible for providing direct care and support services to adults managing mental health challenges. Key responsibilities include conducting assessments, providing case management and skills training, coordinating with interdisciplinary teams, and ensuring clients receive appropriate referrals and follow-up care. Additionally, the role involves maintaining accurate documentation, creating individualized care plans, and fostering a supportive environment aimed at achieving recovery and wellness for individuals under care.
Qualifications
• Strong understanding of Mental Health Screening, Admission, and Referral Services
• Proficiency in conducting Counseling Services, Crisis Intervention, and Case Management
• Knowledge of Medication Management, Skills Training, and Vocational Support
• Experience in collaborating with interdisciplinary teams and providing Crisis Residential or Inpatient Services
• Effective documentation, communication, and organizational skills
• Relevant educational background, such as a bachelor's degree in Psychology, Social Work, or a related field
• Licenses or certifications such as LPC, LMSW, or related credentials preferred
• Prior experience working with adult mental health populations or healthcare services is a plus
