Job Description

• Provide training and education to family/caregivers and other professionals.
• Work collaboratively with professionals from all other disciplines in the SNF setting.
• Appropriately and effectively complete documentation.
• Screen residents and individuals who may benefit from therapy.
• Communicate patient status and needs effectively to the patients, family, caregivers, or other members involved with patient care.
• Expand and enhance self, in both clinical and professional knowledge through associations and continuing education.
• Provide in-service training sessions or other educational presentations to members of the interdisciplinary team as needed.
• Support and participate in department operations/meetings.

Requirements are as follows:
• Licensed in the state of Illinois as a Speech-Language Pathologist.
• Ability to communicate with patients, families, supervisors, and colleagues effectively and professionally.
• Comfortable with computers and computer software.
• Preferred experience working in a SNF, but we are open to other practice area experience.

Responsibilities

  • We are looking for an ST to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders in the adult patients at this site
  • Evaluate, treat, and diagnose speech, language, communication, and swallowing disorders
  • Provide training and education to family/caregivers and other professionals
  • Work collaboratively with professionals from all other disciplines in the SNF setting
  • Appropriately and effectively complete documentation
  • Screen residents and individuals who may benefit from therapy
  • Communicate patient status and needs effectively to the patients, family, caregivers, or other members involved with patient care
