Responsibilities:

1. Performs initial interviews and comprehensive clinical assessments.
2. Assists with developing service plans/person centered plans if applicable.
3. Submit comprehensive clinical assessments within 72hrs of the initiation date.
4. Implements, evaluates, and modifies service plans for all assigned consumers
5. Performs individual, family, group counseling, and crisis intervention for families and consumers.
6. Performs psycho-social assessments.
7. Assists with formulating and coordinates discharge and after care planning.
8. Attends all required clinical meetings.
9. Attends all assigned individual and group supervisions.
10. When required, provides supervision to program staff.
11. Participates in regular professional development in accordance with discipline and level of skill.
12. Is responsible for the proper and timely submission of documentation of all information required by the Department of Mental Health and Substance Abuse Services and Youth Extensions, Inc.
13. Actively coordinates case management, discharge, and aftercare planning and referral to agencies as necessary.
14. Assumes on-call responsibility as necessary.
15. Completes all other work related tasks assigned by the Program Director or the Clinical Director.

Qualifications

  • The clinician shall have a doctor’s or master’s degree in social work from college or university accredited by the Council on Social Work, Education, or a field related to social work, such as sociology, psychology, education or counseling, with a student placement in providing casework services to children and families
  • One year experience in providing casework services to children and families may be substituted for a student placement
  • The Clinician position at a minimum must be qualified mental health professional
