Speech Language Pathologist (SLP) - [Cherokee, IA]
Blue Stone Therapy is seeking a Speech Language Pathologist (SLP) to provide therapy services to adult and geriatric patients at [Cherokee Specialty Care] in [Cherokee, IA].
This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The Speech Language Pathologist is responsible for assessments, care plan development, and implementation of treatment programs addressing dysphagia, swallowing disorders, cognitive-communication deficits, voice disorders, and speech and language impairments.
Perform speech therapy evaluations and develop individualized plans of care
Provide skilled speech language pathology services focused on functional and measurable outcomes
Participate in discharge planning and development of maintenance programs
Supervise Speech Language Pathology Assistants where applicable and in accordance with licensure standards
Complete timely, accurate documentation to support quality care and reimbursement requirements
Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes
Master’s degree in Speech Language Pathology from an accredited program
Current Speech Language Pathologist license (or eligibility for licensure) in applicable state
Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred
Commitment to patient-centered care and professional standards
