ESSENTIAL FUNCTIONS:
• Evaluates and performs assessments of swallowing, speech, and language to identify treatment plans and avenues for individuals
• Proactively creates individualized treatment plans and ensure the individuals carries out their treatment plans by setting goals and objectives addressing specific functional needs for individuals
• Demonstrate to individuals how to make sounds while improving their voices and maintaining fluency
• Provides support, counsel, advice, training and education for individuals, family, caregivers and other professionals
• Efficiently assembles extensive documentation on individuals' progress, care services and therapy adjustments, when necessary, that aligns with standard medical documentation and facility protocol/procedures documentation
• Collaboratively work with other healthcare professionals to provide comprehensive care
• Must display excellent active listening skills, critical thinking skills, verbal/written communication skills, time management skills
• Must display compassion, integrity and respect to all individuals
• Supporting additional responsibilities and/or tasks as needed for the department
PHYSICAL REQUIREMENTS:
• Physically, be able to lift, transfer and assist patients while providing therapy interventions during physical activity
• Must be able to lift and carry items up to 25lbs, including assisting individuals when needed
• Ability to visually be able to perform job duties, including examining individuals and reading information from printed sources and computer screens
OTHER REQUIREMENTS:
• Education: Must be a graduate of an accredited Speech Language Pathologist therapy program
• License/Certifications: Must hold an active and in good standing Wisconsin Speech Language Pathologist License and CPR Certification
• Follows/adheres to organization, federal, and state regulations and guidelines, including Resident/Client rights
