Job Description

Responsibilities:
• Provide individual psychotherapy
• Conduct hybrid in-person/virtual sessions in SNF/ALF/RehabOutpatient settings
• Collaborate with psychiatry and medical providers to support integrated care plans
• Support patients with depression, anxiety, adjustment disorders, and behavioral symptoms related to medical conditions
• Engage families and caregivers as appropriate
Schedule:
• Hybrid (in-person + virtual)
• Flexible structure with a mix of facility rounding and telehealth sessions
Qualifications:
• Licensed in North Carolina (LCSW, LCMHC, or LMFT)
• Experience in medical, geriatric, or post-acute settings beneficial
• Comfortable working independently in a hybrid care model
